What is Hospice?

Hospice is a special "concept" of care provided to patients and families facing a terminal illness. Our Hospice program expertly and seamlessly provides comfort care rather than curative care for your loved one.  

At Memorial Healthcare, our goal is to maintain the patient's quality of life by focusing on supportive care for the terminally ill patient and their family. The patient and family/caregiver are included in the plan of care with attention given to the emotional, spiritual and practical support needed and upon the expressed wishes of the patient and family.

Memorial Healthcare provides Hospice services wherever the patient calls home, whether that is a private residence, an assisted living, or nursing home. Our focus is your personalized journey. Our nursing staff is available 24 hours a day, 7 days a week to provide symptom management.

 

Our Hospice Team

Hospice care is provided by a local interdisciplinary, medically directed team of compassionate and caring professionals consisting of:

  • Hospice Nnurses
  • Home Health Aides
  • Social Workers
  • Spiritual Advisor
  • Hospice Volunteer Services
  • Therapy Services

Your chosen physician is always a part of the team. Hospice is provided without regards to the ability to pay for the care.


Bereavement Services

We understand that grief can be consuming. At Memorial Healthcare we continue our bereavement support, free of charge, for 13 months after discontinuance of Hospice for our Hospice patients' friends and family.

Trained professionals provide bereavement counseling visits, calls and mailings. Separate bereavement support groups, for adults and children, are also offered. Memorial Healthcare Hospice also offers monthly bereavement groups to the community, even when the patient was not on Memorial Healthcare's Hospice services.
