What is Home Health?

The goals of Home Health Care are to promote and maintain health and prevent re-hospitalization, thus reducing the cost of health care. Skilled care is provided in the home on an intermittent basis. Our staff personalizes your care to meet your individual rehabilitation goals. Our skilled medical social workers assist and educate patients and families on available community resources personalized to meet their needs.
